Evaluation of Cortical Brain Parenchyma by Diffusion and Perfusion MRI Before and After Chronic Subdural Hematoma Surgery.

چکیده
AIM To evaluate microcirculatory changes in neighboring parenchyma as a result of pressure due to chronic subdural hematoma (CSDH) in early and late periods after hematoma drainage. MATERIAL AND METHODS The subjects comprised 25 patients who underwent CSDH drainage. Brain diffusion and perfusion MRIs were obtained preoperatively, 48 h (early period) and 2 months (late period) postoperatively. Measurements were performed on 1 cm2 regions of interest in the neighboring parenchymal tissue. RESULTS The early postoperative diffusion values showed improvement compared to the preoperative values. The late postoperative values showed improvement compared to the preoperative and early postoperative values. The early postoperative perfusion values showed slight decline compared to the preoperative values. However, the late postoperative values showed improvement compared to the preoperative and early postoperative values. CONCLUSION The fact that there was an increase in diffusion values from early to late postoperative periods, compared with the preoperative period, indicates that the beneficial effects of surgery increase over time. Brain perfusion was found to be slightly decreased in early postoperative period. Following CSDH drainage, neurological deteriorations are observed in some patients in early postoperative periods; slight impairment in perfusion may account for this. However, during late postoperative period, perfusion was seen to recover prominently.

متن کاملبررسی و درمان آبسه مغزی Evaluation and treatment of Brain Abscess)
ABSTRACT: Brain abscess is a localized parenchymal infection of the brain , which may develop from three sources:(1)by direct extension of preexisting infection in the paranasal and mastoid sinus.(2) from cranial trauma,(3) by hematogenous spread. Brain abscess occurs more frequently in the young and in males. With more common utilization of anaerobic cultures,it has become apparent that a gr...
متن کاملCorrelation between level of consciousness and some cT findings in chronic subdural hematoma
There is little information concerning the correlation between clinical and CT findings in chronic subdural hematoma . This study was performed on 69 hematoma patients to find correlations between CT findings and GCS. Ninety one percent of patients were male and 9℅ were female. The average age was 63. In 87℅ of cases, the lesions was unilateral and in the remaining 13℅ , it was bilateral. The r...
متن کاملChronic Subdural Hematoma Infected by Propionibacterium Acnes: A Case Report
We present a very rare case of a patient with an infected subdural hematoma due to Propionibacterium acnes. A 63-year-old male complained of dizziness and was admitted to our hospital. He had a history of left chronic subdural hematoma due to a traffic accident, which had been conservatively treated. Physical, neurological and laboratory examinations revealed no definite abnormality. Plain CT s...

ABSTRACT Background: High resolution CT scan has made early diagnosis of intraventricular hemorrhage (IVH) easier. Posttraumatic intraventricular hemorrhage has been reported to a greater extent because of the CT scan. Methods: 904 patients were admitted in the NSICU from March 2001 to March 2002 with severe closed head injury, of those only 31 patients with intraventricular hemorrhage (GCS le...
